Centerville Nursing and Rehab receives five star rating in February

Centerville Nursing and Rehab received a five star rating this month for the second quarter in a row from the Center for Medicaid and Medicare Services.  

The rating is derived from three different categories that are combined for an overall rating: staffing, health inspections and quality measures. Each category is a snapshot at a specific time and then rated in comparison to all nursing homes in the nation.  

The health inspections review the last three years of annual and complaint investigations completed by each states investigative department. In Iowa it is the Department of Inspections and Appeals.

The most recent years are weighted heavier than later surveys.  In 2012 Centerville Nursing and Rehab received only one minor B level tag for paperwork compliance. No complaint surveys were filed with the state and the facility received an Excellent Care Award for 2012.   

Quality measures is made up of the residents care levels that are assessed by the facilities care plan coordinator. The information is transmitted to CMS in a report called Minimum Data Set.

The facility also received a five star rating in this category.  The last rating is for staffing and the facility received a three star rating for nursing services.  

Once the ratings are compiled nursing homes are then rated in comparison to all the other nursing homes in the nation. The top 80-100 in percentage receive a five star rating, and the ratings decrease a notch for each consecutive 20 percent drop until a one star rating is for all facilities from 1-20 percent.   

The reports are released in February, May, August and November.

The facility had the highest rating of all nursing homes in a 50 mile radius.   

The facility also had the highest resident satisfaction in the state of Iowa. Residents, their families, or power of attorneys fill out an annual survey. The average score was a 4.7 out of 5.0 scale. The 2012 data was from the 2011 data. A new report for the 2012 data should be out by the middle of April.
